Note: Students are those who wish to have a mentor take them out hunting during the camp. Mentors are those who have some experience hunting and wish to help a hunter during the camp (mentors will not have a tag). Mentors do not have to know the unit (we have places we can send you), but mentors have to know how to hunt safely and be able to teach a student how to hunt.

Participation in any Team Young Gunz (TYGZ) activity and use of any recreational facilities involves a risk of injury despite all safety precautions. Having been informed of the activities conducted by TYGZ, I, as an individual, assume all risks and hazards incidental to the activities, and release from responsibility and agreed to indemnify and hold harmless TYGZ, its officers, directors, independent contractors, volunteers and all employees for any illness or injury to me or my family members occurring during his/her/our participation in any activity or use of any recreational facilities at or conducted by TYGZ.

I certify under penalty of perjury that the foregoing is true and correct and agree to all terms stated in the above contract/application. I understand that TYGZ may remove me from or refuse service for this event if TYGZ discovers that I have not answered truthfully to any of the above questions and terms.
